Checkin and Checkout time:When boarding your pet must be picked up before 1p to ensure no other boarding charges are applied. A pickup between the hours of 1p and 4p will result in ½ day charge and a pickup after 4p will result in a full day charge. *

VACCINATIONS: The following vaccinations are required to be current prior to boarding: Rabies, Distemper/Parvo, Leptospirosis, Canine Influenza and Bordetella. If vaccines are not current, they will be administered at the owner's expense. Proof of vaccinations will need to be submitted prior to the boarding. If proof of vaccinations is not submitted by the client/owner prior to, or at the time of the boarding reservation, Erlanger Pet Resort and Day Spa reserves the right to administer or have administered all required vaccinations at the owner’s expense. *

INTESTINAL PARASITES: All pets are required to have a negative fecal test for intestinal parasites within one year of boarding. Fecal results from another veterinary clinic must be provided at the time of boarding. If not current on intestinal parasite control or no current negative fecal test, pet will be tested at the Erlanger Veterinary Hospital and dewormed if necessary, at the owner's expense. *

FLEA CONTROL: Any pet that is admitted for boarding that is infested with fleas and/or ticks will be treated at the owner's expense. *

CONSENT FOR TREATMENT: In cases of illness or injury, I, the undersigned, do hereby give my consent for the veterinarians of Erlanger Veterinary Hospital to treat, prescribe for, or operate on my pet(s) while they are boarded at Erlanger Pet Resort and Day Spa. I am responsible for all fees incurred by such services. In a situation where I or my emergency contact cannot be reached, I authorize the following. *

I authorized a certain amount of emergency money for medical treatment
Please treat my pet at the veterinarian’s discretion with no financial limit

For after hours veterinary care we use the Veterinarian Emergency Group (VEG). Transportation fees of $100 will be charged for each time the Erlanger Pet Resort and Day Spa transports your pet to or from the emergency clinic or a veterinarian other than the Erlanger Veterinary Hospital. *

Erlanger Pet Resort and Day Spa and it's employees will use all reasonable precautions against illness, injury, or escape of my pet, but will not be held liable or responsible in any manner, or circumstances, on account of care, treatment, or safekeeping of my pet, as it is thoroughly understood that I assume all risks. *

Should circumstances arise that my pet remains unclaimed after the date, which I have stated as the pick-up date, I understand that written notice will be mailed to the address above. Seven days post written notice, the pet(s) will be considered abandoned and may be rehomed, or taken to a humane society, as Erlanger Pet Resort and Day Spa deems best. It is further understood that such action will not relieve me from paying all costs of service and use of the hospital, including all costs of boarding. *
